North Wasco County School District Athletic Department uses FinalForms to manage athletic participation forms online. This platform makes it easy for parents to complete, sign, and update forms—all while saving time by storing information year-to-year.
All parents of student-athletes are required to register and complete forms through FinalForms.
Students interested in participating in athletics must also have a current physical on file with the athletics office by the start of their first practice every season. English Form/Spanish Form
